If your medical registration board or other organization requires EICS verification of medical credentials, the board will provide you with its customized EICS application materials. To access applications for the following boards, visit their websites.

	College of Physicians and Surgeons of Alberta - The EICS application for the College of Physicians and Surgeons of Alberta is available through its website.
	College of Physicians and Surgeons of Nova Scotia - The EICS application for the College of Physicians and Surgeons of Nova Scotia is available through its website.
