Values History Project
You will receive two forms from the professor, a values advance directive and
Pennsylvania's sample living will..
- After considering the questions, complete the values focused advance directive
form.
- Show your form to a friend or family member and ask for his/her feedback.
You may choose to change it or not.
- Based on your final version of the values focused advance directive, fill
out the sample Pennsylvania living will form.
- Write a short two-part essay, typed, double-spaced.
In the first part, answer the following questions:
• What were your feelings/emotions when discussing the form?
• How did your friend/family member react to what you wrote?
• What questions or comments did he/she have?
• How did you respond to those comments?
• Do you think this experience was typical of discussions about end
of life issues?
- In the second part of the essay comment on the relative merits of each form
for communicating your care wishes and protecting your interests once you
lose decisional-capacity.
- On date due, participate in the class project where you will discuss your
advance directives and your views about advance directives in general.
- The paper and discussion are due at the beginning of class on March 28.
You do not need to hand in the completed living will or the values focused
advance directive.
